To monitor an Azure SQL server using SQL Diagnostic Manager, you have to create a SQL Server user on the instance with the following minimum permissions:

Azure - Minimum permissions

  • db_datareader
  • View Definition
  • View Database State
  • Select
  • Execute
  • Connect

Azure - Recommended Permissions

  • Minimum permissions plus Control
Recommended permissions are on database level.
